Fees & Insurance


CRT is committed to serving all patients, regardless of their race, color, sex, national origin, disability, religion, or sexual orientation. Patients will be provided with services, regardless of their ability to pay. We offer a sliding scale fee for individuals who require financial assistance, as well as payment plans. Treatment services are also provided through various grants and foundations for those who qualify. The Request for Financial Assistance Form is available through our Intake Coordinator.

All therapy Services provided through CRT's community partner sites are provided without a cost and are grant funded.

Therapy

All therapy Services provided through CRT's community partner sites are provided without a cost and are grant funded.

The fee for individual, dyadic, and family therapy sessions is $175/hour. Group therapy rates are $85/hour. Our therapists are also in-network with BCBS PPO, United HealthCare/Optum, Cigna, and TRICARE. We will work with you to submit claims to other out-of-network health insurance providers, though we do not bill those therapy services directly. At the time of your intake, we will provide you with further information regarding out-of-network insurance billing. Additionally, CRT maintains contracts with the Illinois Department of Children and Family Services (DCFS). We provide therapy services for children while they are in the foster care system and after their adoption. For more information regarding the services provided through DCFS, please contact our Intake Coordinator.

Comprehensive Evaluations

Most of the comprehensive evaluations conducted at CRT include developmental, psychological or neuropsychological, and medical components.  Each evaluation entails an initial interview, test administration, and a physical examination.  We also provide feedback on the day of the evaluation, and we prepare and send a written report once the evaluation is complete.  The initial interview costs $275.  We bill test administration, the feedback session, and report preparation at an hourly rate of $200.  The physical examination costs $250.

Families have several payment options.  If your child is in foster care or was adopted through DCFS, our contracts with DCFS may cover the costs associated with his or her evaluation.  Other funding and payment plans are also available, depending upon a family’s unique situation and completion of appropriate paperwork.  Additionally, our psychologists are in-network with BCBS PPO, and we are happy to work with you if you decide to submit out-of-network claims directly to your insurance company.  We can bill Medicaid or BCBS PPO for the physical examination.  For more information, please contact our Intake Coordinator. 

Medication Management

CRT's pediatricians provide medication management services for children with ADHD. Telephone consultation is available for established patients in appropriate circumstances. Medication management services can be billed directly to Medicaid. Families with other insurance will be provided with a detailed bill to submit to their insurance company. A sliding scale program is available for those without insurance as well. Fees are determined based on the complexity of each visit — a detailed schedule of fees can be accessed by request.

Pre-Adoptive Consultation/Education

Pre-Adoptive Consultation/Education, which includes a detailed review of any prenatal drug/alcohol exposure, family history, photos, and videos, costs $450.


Consultative Services

CRT staff is available for consultation via telephone, secure video conference, and for educational or other meetings. These services are provided at a rate of $200/hour.
