Camper Medical Forms for Accepted Campers & Counselors
After you receive notice that you/ your child has been accepted to Camp SOAR 2023, please download and complete the necessary pages for Medication & OTC Medication Authorization here:
Additional items needed are Immunization Records, COVID-19 vaccination card, & any Seizure Plans, Behavior Plans, or G-Tube Plans. Documents will be uploaded to your child's CampBrain application.
